Vitamin E works as a free radical scavenger, protecting polyunsaturated fatty acids (PUFA), a major structural component of the cell membranes, from peroxidation. In the past few decades, there has been increasing interest in the role of free radicals and anti-oxidants in atherosclerosis and carcinogenesis. Deficiency of vitamin E has been connected to cardiovascular events. Low-density lipoprotein (LDL) plays a central role in these hypotheses. When LDL is exposed to oxidative stress, it undergoes a cascade of changes affecting the vascular endothelium, thereby facilitating atherogenesis. This theory is referred to as the oxidative modification hypothesis, and has fueled epidemiologic studies and clinical trials in cardiology to determine the role of antioxidants, such as vitamin E, in prevention and treatment of atherosclerotic cardiovascular disease.
